This service includes:\n<ul>\n \t<li>Update of standard vaccinations (if required)<\/li>\n \t<li>Travel vaccinations (including Yellow Fever)<\/li>\n \t<li>Travel medications (e.g., antimalarial)<\/li>\n \t<li>Refills of certain maintenance medications needed for travel<\/li>\n \t<li>Counselling on disease prevention and management<\/li>\n<\/ul>\nYou can begin the booking process for consultation on-line by completing a travel questionnaire. Please give yourself <strong>at least 6-8 weeks before travel<\/strong> to have the consult done. <strong>You will need to know your vaccination history.<\/strong>\u00a0<strong>If possible, bring in your yellow card or the information from your Public Health. <\/strong>For vaccination histories, if you went to school in Canada &#8211; you should be able to contact the public health authority where you went to school. A record of vaccinations is required to attend school.\n\nPlease click\u00a0<a title=\"Travel Questionnaire\" href=\"http:\/\/www.prescriptionshop.ca\/rxshop\/?page_id=341\">here<\/a>\u00a0to complete the questionnaire on-line.\n\nThe fees for travel consultations can be found\u00a0<a title=\"Fees\" href=\"http:\/\/www.prescriptionshop.ca\/rxshop\/?page_id=150\">here<\/a>.\n\n\n\nPlease ensure you bring your insurance information with you (the card itself, or a picture of the card front and back) to the appointment if you are a new patient so that we can bill any vaccines or medications through your insurance plan(s).\n\nRemember to book the <strong>vaccine administration appointment<\/strong> with the nurses at <strong>Carleton <\/strong><strong>Health Services<\/strong> for any administration of prescribed vaccines. Administration of vaccines is not done during the consult.\n\n \n\nIn terms of timing: <strong>Yellow Fever vaccine<\/strong> should be done at<strong> least 10 days before entry<\/strong> into the country of question and requires you to bring your <strong>passport<\/strong> for administration alongside the vaccine. Please click <a href=\"https:\/\/www.immunize.org\/vaccines\/vis\/yellow-fever\/\">here<\/a> for information about the Yellow Fever vaccine. <strong>Meningococcal vaccine<\/strong> for Hajj or Umrah must show that the vaccine was administered <strong>at least 10 days before arrival in Saudi Arabia<\/strong> and is valid for 3 years for the polysaccharide vaccine, or within the last 5 years for the conjugate vaccine (we don&#8217;t carry the polysaccharide vaccine).
